Print Instructions

After you open the form in your browser, you have to print it, fill it out, and send it to Dan Gamelin at the address on the form. To print the form properly, check the "print as image" box in the print dialogue window (this is the window that opens when you click on the print button). Typically, the "print as image" box is located in the upper right hand corner of the print dialogue window.

SPECIAL NOTE: Orders should be mailed to Dan Gamelin. PLEASE DO NOT send any of your orders to the Association address in Clawson, Michigan. Doing so will result in a delay in processing your order and creates more work for the Association members involved.

Ordering Restrictions

Commemorative Medallions are limited in availability. Individuals eligible to receive one Medallion include former employees of the Air Proprietaries of the Pacific Corporation, the spouse of an employee KIA or MIA, or one orphaned child of an employee KIA or MIA. Other family members of surviving employees are not eligible to receive Medallions.

Anyone can order Postal Caches or CIA Citations.

To place your order, please use this ORDER FORM. Follow the print instructions above to ensure the form prints properly. Be sure to mail this form to Dan Gamelin at the address on the form.
